Retain definitions as well as corrections
A correction is easier to reuse when the underlying term, population, period and calculation remain explicit. Related inpatient service activity, HRRP eligible discharges and completed follow-up are different concepts. Local definitions may later add organization-specific meaning, but they should not silently replace the public definition. A reviewed interpretation records what changed, why, and which questions still need local confirmation.
When the same exception must be re-explained to several people or systems, the cost is not only delay. The organization loses the evidence needed to distinguish a one-time issue from a recurring operating pattern.
An exception can be treated as work to clear, or as evidence about how the operating system behaves. A correction can overwrite a field, or reveal that a source, assumption, route or decision needs to change. Lime Health treats both as candidates for governed learning—not automatic instructions to the next agent.
Repeated operating observations becomes a domain asset only when its provenance, disposition and reuse authority remain visible.
Reduce repeated reconstruction of the same exception
Continuing the synthetic referral example. Several reviewed cases show that coordinators repeatedly reconstruct acceptance status from messages. The useful learning candidate is a bounded process question: would an explicit acceptance checkpoint and named owner reduce that rework? The cases do not establish the cause of a readmission or justify automatically changing care.
Before adopting the candidate, a reviewer checks whether the apparent pattern reflects missing receipts, delayed recording, a capacity issue or a sampling artifact. The team agrees what evidence would distinguish those possibilities. A proposed change then receives one of four dispositions: accept for a bounded evaluation, amend, defer for evidence or reject.
If the change is promoted, retain its version, scope and rollback condition. Compare reconstruction time, repeated contacts, unresolved handoffs and added reviewer burden. A correction that improves traceability but adds unsustainable work should remain open to reversal.
This gives operations a reusable lesson with an owner and a boundary. The technical lifecycle below explains how to keep that lesson traceable rather than allowing an unreviewed anecdote to become policy.
First classify what changed
Suppose public research assigns a hospital to the wrong system, a quality owner corrects an operating assumption, or repeated scheduling exceptions show that the normal route does not fit a rural referral pattern. These events are different and should not produce the same mutation.
| Observation | Likely diagnosis | Governed response |
|---|---|---|
| Existing evidence was available but missed | Procedure or routing fault | Correct the route or procedure; keep the knowledge fact stable |
| Required evidence was absent | Knowledge gap | Add or amend a sourced candidate and its retrieval route |
| Sources or experts disagree | Contradiction or ambiguity | Hold the candidate; route to named human authority |
| Source published a newer effective state | Supersession | Retain prior lineage and activate the approved successor |
| Workflow exception recurs under similar conditions | Pattern candidate | Evaluate frequency, impact and boundary before promotion |
Review a lesson before reusing it
First capture the observation and its source. Then compare it with the current definition or process, consider alternative explanations, and ask the responsible reviewer what should change. Retain the decision, affected scope and date before using a revised interpretation.
In a later workflow trial, the same approach can distinguish a missing acceptance record from delayed documentation or a genuine ownership gap. A repeated anecdote is a question to examine, not a new policy. Check whether the change adds work elsewhere before adopting it.
Versioning, independent review and rollback are supporting design requirements. The current brief process can retain checked corrections; it does not automatically train a model, modify hospital procedures or demonstrate continuous customer learning.
Corrections should raise the future bar
A confirmed correction should join the regression suite. If a facility identity was previously misattributed, future releases should test the identity and relationship rule. If a route missed an applicable policy, the test should verify that the policy is discoverable and selected under the same conditions.
This makes the system more dependable without retraining a model after every change. Knowledge remains readable, versioned and reversible. Models can assist with candidate extraction or evaluation only inside their admitted semantic role.
Why this matters to the operating team
Public corrections can build trust before a customer relationship. Discovery dispositions make workflow assumptions more precise. workflow trial exceptions reveal which operating conditions matter. Production receipts and outcomes show whether the promoted guidance remains useful. Together, these can give the operating team a reviewed account of what was tried, what changed and when a prior lesson should no longer be used. Customer evidence remains scoped to its permitted purpose; reuse does not imply cross-customer sharing.
The same evidence can make human burden visible: how often teams reconcile the same issue, how long exceptions wait, which decisions return for clarification and where ownership repeatedly changes. Lime Health can measure those patterns. A claim that the platform reduces burden requires an agreed baseline. It also requires a comparison period and balancing measures.
Invite corrections early, but never allow a correction to become active knowledge without source, review, impact analysis and an effective release.

Financial correction lineage
A corrected HCRIS report, changed CMS value, resolved CCN or calculation-state change creates a versioned successor with impact evidence. The prior scenario remains reconstructable. Downstream briefs and ranks change only through validation and promotion, and missing values never silently become zero.
Boundary
This is a proposed learning method supported by versioned reference foundations. Public-data correction review exists separately from future customer operating workflows. No clinical benefit, workload reduction or production learning result is claimed here.
