About Lime Health

Lime Health connects public healthcare insights with local operational knowledge to help care teams make post-discharge transitions more reliable, visible and easier to coordinate.

Care transitions cross departments, facilities and organizations. When ownership and follow-up status are recorded separately, care teams have to piece together who accepted a handoff, what remains open and whether the work is complete. This fragmented tracking can mean repeated status calls, duplicated effort and delays in care after discharge.

Our focus

We connect public healthcare data to help care teams understand their facility’s context, compare reported readmission results and identify care-transition questions worth discussing. Local operational knowledge then helps determine where scheduling, post-discharge referrals or medication follow-up could improve.

Designed to work alongside existing systems, Lime Health makes handoff acceptance, ownership and follow-up status visible across departments and care settings.

The benefits we aim to deliver:

Our team

Lime Health is founded and led by Prasada Martala, whose background is in life-sciences data and AI. We bring a data perspective to care coordination and welcome the clinical, operational and technical expertise needed to make the work useful.

Our values

Put people first: Improve coordination in ways that respect patients, caregivers and the teams supporting them.

Improve through collaboration: Bring care teams, departments and organizations into a shared exchange with Lime Health—combining perspectives, shaping improvements and learning together.

Build trust through transparency: Ground our work in data, make sources and reasoning checkable, and be clear about what is known, what remains uncertain and what changes when we learn more.
